Medicare Advantage Private Fee-for–Service (PFFS) Plan

Please note, a Medicare Advantage Private Fee-for–Service (PFFS) plan works differently than a Medicare Supplement plan. Your doctor or hospital must agree to accept the plan’s terms and conditions prior to providing healthcare services to you, with the exception of emergencies. If your doctor or hospital does not agree to accept our payment terms and conditions, they may not provide healthcare services to you, except in emergencies. With any Medicare Advantage plan, you must continue to pay your Medicare Part B premium unless otherwise covered by Medicaid or another third party, along with your plan premium (if any).

You must be enrolled in Medicare Parts A and B, and live in the plan’s service area and not have been diagnosed with End-Stage Renal Disease (ESRD) to be eligible.
